The
SD-KHAN Community Collaborative Meeting is composed of
over sixty member organizations with broad representation
form community based organizations, hospitals, health plans,
health care providers, government agencies, schools, and churches.
The SD-KHAN Community Collaborative developed workgroups including
Community Collaborative, School Workgroup,
Healthy Families Enrollment Retention. Each workgroup
developed comprehensive outreach to provide outreach to targeted
locations with the ultimate goal of improving access to health
services for low and middle income, uninsured children.

Provide outreach through collaboration with community-based
and faith-based organizations serving families with children
who may need health insurance and who are willing to assist
with outreach activities.

Healthy
Families Enrollment Retention
The Healthy Families Enrollment
Retention Workgroup is a collaborative committee dedicated
to developing a plan of action for keeping San Diego’s
County’s children enrolled in Healthy Families. Strategies
developed include local educational messages and advocacy
for statewide systems-based changes to reduce those Healthy
Families disenrollments due to possibly avoidable reasons.
As a result, it is the hope of the committee that more children
will maintain and utilize their medical home.
