GENERAL INFORMATION


San Diego Kids Health Assurance Network (SD-KHAN) shares the American Academy of Pediatrics Community Access to Child Health (CATCH) vision that every child in every community has a medical home and other needed services to reach optimal health and well being.

To achieve this goal SD-KHAN is in the process of developing "Force of 100", a health care network of 100 caring physicians to provide free and/or low-cost medical care to one or more children with no other source of care.

Many of you are already providing services, and we would like to acknowledge your efforts by adding your name to the SD-KHAN "Force of 100" list.

We invite you to become a member of the SD-KHAN "Force of 100" team. As a participating SD-KHAN provider, you can:

Choose the number of patients you wish to serve.
Withdraw at any time and discontinue care by giving a 30-day notice to patients
and SD-KHAN
